In most cases abbreviations have been expanded without comment to avoid confusion. A new page numbering system is used based on the microfilm frame order. Ince's original page numbers and the DLSL photocopy reference are given in red brackets. The original pages are held at DLSL, are separated, and unfortunately do not possess a coherent numbering system.

On neither is written a contemporary date, source nor reason for duplication. 064d was written first, by an as yet unidentified hand. 064c was written by Ince and was fixed on top of 064d, attached at the top left-hand corner. This is visible on the film and has been confirmed to us by the librarian at Derby LSL. John Palmer says: In my opinion, 064d was written first by a junior in Ince's office.
